Seven questions. No wrong answers.

What follows is not a standard intake form. Each question is a diagnostic instrument. Your answers become the first layer of the structural read — they tell us what your current map looks like before the session begins to shape it.

Answer precisely rather than extensively. The most useful answers are often the most direct. Write what you actually think, including if you are not sure.

This is a private document. Your answers are read as structural data, not evaluated or judged.

Allow 10–15 minutes. Complete in one sitting if possible.
